Welcome to The Food Allergy (FA) Passport™!
We hope that this personalized tool helps caregivers and children manage their food allergies more easily and with greater confidence.
Sharing the completed FA Passport™ in situations such as daycare, when children are with a new caregiver or babysitter, or when they sleep over with a friend or relative, can be helpful. After you complete the FA Passport™, it can be printed or sent via email.
The completed FA Passport™ lists the child’s specific food allergies, and the location and names of important lifesaving medication. It provides contact information, safe and unsafe foods, describes specific food allergy symptoms, and shows how to properly read a food label to prevent accidental exposure. It also includes an Emergency Action Plan (EAP) that shows how to use an epinephrine autoinjector in the case of an allergic reaction.
